Italian Tube crash

One person has died and another 60 have been injured - five seriously - in a crash on Rome's underground. A train drove into the back on another which was stationary in the platform.

While officials work out whether the cause was human error or a technical problem with the signalling system, Tubeworker asks why, even if it was human error, the signalling system did not stop the train.

Our thoughts are with the victims and their loved ones.
